  There are several downloads on this site, including an application form as well as a form to appeal decisions about eligibility. The site also provides tools to help individuals find out if they quality for LIS, or find other programs to help purchase prescriptions. There are links to additional web sites with calculators and other tools designed to help people make decisions when it comes to managing Medicare Part D prescription drug costs.
