Plan Formularies May Change as of January 1, 2007

Patients should review their Plan’s complete 2007 formulary for their currently prescribed medications. If the Plan has dropped a required medication from their formulary or changed the Utilization Management requirements, etc. for 2007, the patient has the option of filing a request for an exception or changing to a Plan that does cover them.

Note: The ANOC (Annual Notice of Coverage) mailed at the end of October is not beneficiary specific and it will be necessary to contact the Plan directly for the details. CMS has improved the tools to assist beneficiaries’ review of Plan choices (www.medicare.gov) but we continue to recommend that you contact the Plan for confirmation.
