CMS has developed a dedicated page that contains information on formulary and exceptions resources for physicians.  Follow the link to the CMS Part D Formulary Information for Physicians page

At the bottom of that page you will find the following downloads:

• Exceptions and Appeals Contact Information (Excel zip file with contact names, addresses, phone/fax numbers and email addresses where available).  This information is intended to be used as a last resort if what you need is not readily available from the plans themselves. Note that in this Excel file, Sheet 1 provides contact information for Exceptions and Sheet 2 lists those for Appeals.  The file is updated as information is received so you should routinely check for updated information.
• Medicare Prescription Drug Coverage Provider Communication Form (PDF file).  Model template/optional fax form for communication between pharmacist and physician to obtain information for prescription information or changes.  Note: CMS is not mandating the use of this fax form.
• Medicare Prescription Drug Coverage Determination Request Form and Instructions (PDF file).  Model template/optional fax form “can be adopted/and or modified to request coverage determinations.”  Note:  CMS is not mandating the use of this form.

NOTE: The model exceptions form has limited utility inasmuch as plans may vary in their information requirements. We are developing more appropriate instructions about exceptions request that will be posted shortly on the mentalhealthPartD website.
